Do I need to include a Reference Cover Form in front of every reference letter this year?

 I came across a Reference Cover Form from a previous application cycle. It includes sections for the applicant’s details, referee’s details, and a rating chart for skills like teaching, public speaking, leadership, working under pressure, and teamwork.

It also mentions that this form should be stapled to the front of each reference letter.

Does anyone know if we still need to include something like this for the 2024 application, or has the process changed?
