Respectfully, I’ve seen this same, identical question asked at least 20 times here in the last year. Perhaps searching for 1 minute might get you your answer?
If your spouse is not able to be ensured via your employment based insurance as your dependent they’d have to register at cityhall for the vanilla national health insurance. It’s not complicated nor super expensive.
If you know which city you’ll be living in you should be able to find out specific details from the cityhall website. For example, Shinjuku city has this page: https://www.foreign.city.shinjuku.lg.jp/en/kurashi/hokenryo/
