Statement of Physician Question – Is it Acceptable with Lack of Detail?

I want to double-check if my Statement of Physician would be acceptable. (I don't believe it is at the moment.)

My physician only put the date of diagnosis on the Details and Explanation portion of the form without actually providing details and explanations for past medical issues. These cases include things like previous back pain to mental health issues like Adjustment Disorder. All these cases have since been resolved which my physician made note of in the Status portion along with providing details for the Therapy portion such as describing that I took physical therapy for the back pain.

As is, would this form be adaptable or should I go back to have them add more details?
I would also add that they insisted that the way they filled it out was adequate and that the program shouldn't want more details beyond what they wrote. I pressed them to add a bit more and said I would need to come back again in case the current form is acceptable.
