Question about Statement of Physician Mental Illness status(US)

Hey all!

My doctor has not yet gotten back to me with a filled out form. I was going to email them back to ask if they could send me the form back or to see if they had any questions. I did send them the guidelines and filled out my portion. However, looking back at the form, I have a question that I would like some insight on before I send her another email.

I am taking hormones to treat my gender dysphoria. I was getting virtual therapy for anxiety and depression biweekly at first and eventually started only seeing my therapist once a month. I had to end my sessions last month because I lost my health insurance and could not afford paying without it. My therapist thought I was in a good place anyway, because of all my progress and because I moved into my own place and felt more in control of my life.

Anyway, I know she will have to list the frequency of my hormone treatment. She will probably check the status at ongoing. I am not worried about this section. However, will her checking "ongoing" for anxiety and depression be a red flag? Or is the main point the "yes" and "no" boxes at the bottom?

I wouldn't say I'm "cured" of anxiety and depression, but I can manage it just fine. And I have no history of being on medication to treat it. I would imagine "anxiety" would be fine to be checked as "ongoing", but I wonder how "depression" would be counted if it is marked as "ongoing". I was diagnosed with "major depressive disorder" but at this point its mostly seasonal, and I know how to manage it.

I know that she will check "yes" to the question about my adequacy to participate on JET. But because there is a specific way these forms should be filled out, I was wondering if anyone here has thoughts on if these are better checked off as "ongoing" or "recovered/in remission". If she checks them off as "recovered" is that going to stand out as odd? I don't know that these are typically things people "recover" from, they just experience them at a lower level or get better at managing them.

I know there have been JETs with the same or similar diagnoses, but I havent seen anything about how those were marked on the form. I searched the subreddit to see if this has been answered before. It seems like its fine to include in the form, but I don't see anything about the status label.

(In case this comes up in the comments: I have studied abroad in Japan before. I did just fine. I didn't get therapy while in Japan because my therapist can only operate if I am within our state. I know I will have no issues with my diagnoses on JET and my therapist is also confident about that. We discussed this before my last session.)
