I need help (navigating the medical system)

Hi everyone – I need help.

I’ve been sick for almost six weeks now. I’ve lived in Tokyo since June, and I don’t speak Japanese. I’ve gone to three English speaking different clinics now. I went to two, and got an xray, urine tests, and bloods done.

I then tried to show up at two different emergency departments because of how unwell I was feeling, got turned away from one (Juntendo Nerima) because I couldn’t speak Japanese and got turned away from the other (St Luke’s) because no specialists were working. I was advised by a clinic to get a referral when my bloods came back. My bloods came back and were clear except for inflammation, and the doctor wouldn’t give me a referral to the hospital for further tests. He told me he thought it was a weeks long virus or infection that would clear up on its own.

I am still feeling very unwell. It’s been six weeks now of barely being able to do anything or go anywhere. What on earth is the next step? I can go back to that clinic and beg for a referral, but will it even do anything? Will the hospital be able to do anything for me? How do I convince him to give me a referral?
