Relationship questionaire and coe application confusion

Hey everyone,

My fiancée (now wife) and I recently got married in Japan! We met there while she was/still is on a work visa and I was just visiting. Over time, I kept going back to see her(as a visitor for 3 months each time), and since Japan has such sentimental value for us, we decided to get married there.

I’m a freelancer, so I can work from anywhere, while she’s tied to her Japanese company under her work contract. Right now, we’re in the process of applying for a Certificate of Eligibility (COE) for a Dependent Visa so that we can live together in Japan while she continues her job and make the most of our time there.

We’re both foreign nationals from different countries, and I just wanted to clarify something:
Do we also need to submit the “relationship questionnaire” or is that only for the spouse visa applications?

Any advice or experiences from people who’ve gone through a similar situation would be super helpful! Especially with documents suggestions
