Need help with Japanese address

1. You don’t need to put anything in Province line (Because it’s also Tokyo), but I’d enter the rest (everything before Tokyo) in the Address 1.
   I’d also switch the order to Machida-shi, Oyamagaoka 4-9-1, but that’s not necessary.
