Does the statement of physician need to cover every reason I've ever gone to the doctor or just things that I've been diagnosed with/prescribed meds for? It seems insane to have to list every doctor's appointment in my life
I'm a Canadian applicant if that matters
EDIT: Also does anyone know if a counsellor (not a psychologist) counts as a medical professional?

It doesn’t need to be mundane things like if you had the flu, or if you had some mild infection. It is mostly for chronic conditions – things that may affect your day to day in Japan.
No, a counsellor cannot fill out the paperwork. It needs to be a physician. If you do not have a family doctor, a walk-in clinic should be able to do it for you.
If you have questions, your best bet is to contact your consulates JET desk. They are there to help you.
If you are currently being treated for something, put it down. If not, leave it empty.
