Does anyone know how to retrieve kazei shomeisho and nozei shomeisho from shibuya city office?

I live in a neighborhood in Shibuya, does anyone know how to obtain the kazei shomeiso and nozei shomeisho online?

Last year I went to the city office to get them but I'm a little sick these days and bedridden so I would like to apply for my visa extension online, does anyone have an idea or the procedure to get these online?

I tried to use my number portal but I don't understand how to get them still
