Where to get Health insurance and pension payment certificate as a full time employee?

Hi, I’m a full time employee for past few years in my current job. I wonder where I can get Health insurance payment certificate and pension payment certificate since it’s deducted from my payroll. I want last one year of record for each.

Is this something to get from my local ward office? Or pension office ? Or ask company ?
