It seems that the main requirement is a chest X-ray, presumably to filter out TB.

The rest is just "History of disease?" type questions. There's no requirement to report physical or blood test results.

There's a specific form provided by the school to fill out, doctor would need to be willing to do that.

I am happy to do this in English or Japanese, though English is preferred.
I need to get this done as soon as humanly possible. Hell, tomorrow would be nice (Though obviously likely not possible.) I do have 国民保険 but not 社会保険。
Any suggestions on where to go within the 23 wards? Exactly location and cost are irrelevant. I have clinics and specialists I go to for stuff, but never had a screening check like this.
