So, I'm confused.
I did the JET program back in 2017, and I recall having my blood taken for the physical. I'm reapplying now, and I don't see any form (besides the Statement of Physician) to give to my doctor before getting my labs done. Is that something you only do after being initially accepted? I can't remember.
I pass out and have seizures every time I get my blood drawn, so this time I asked my doctor for medication to help calm my anxiety, so I gave her the Statement of Physician form to fill out. Is that all I need to send in before November 14th?
