Hi guys! New poster here. I have a question I'd like some help with! I know we need to self-report any medical conditions or diagnoses, but I'm a little confused. Some people on this thread have talked about getting a doctor's note, and others haven't.
To clarify:
As long as you don't mark "Yes" to any of the issues on the form, you DON'T need a physician to fill out the "2026 Statement of Physician Form" on your behalf, right?
