Currently, I am beginning the process of obtaining the affidavit after seeing multiple posts mentioning this topic following the U.S. Embassy and Consulates in Japan discontinuing the issuance of the Affidavit of Competency to Marry.
I am a resident of Kyoto and have already inquired at my local ward office regarding the exact requirements for marriage. So far, they have informed me that the necessary documents include a physically notarized and apostilled affidavit from my partner’s home state, her passport, and finally a 申述書 (Shinjutsusho), all translated into Japanese.
Luckily, my partner has not fully moved yet, and we are planning to complete the filing during her one-month stay in Japan next month.
My question is whether it would be possible to use the old embassy-provided affidavit template, provided that the footer indicating it was commissioned and qualified by the U.S. Embassy is removed.
Below is the URL to the U.S. Embassy in Japan webpage for reference.

Any experience with ward offices and city halls (I understand they differ) would be greatly appreciated.
