Decided to receive a lump sum for my Japanese pension, but had a couple of questions regarding the documents I need to send alongside the form.
③ Please attach a document (such as a certificate issued by the bank) that can verify the “bank name”, “branch name”, “branch address”, “account number”, and “that the account holder is the claimant themselves”. If you wish to receive the payment at a financial institution in Japan, the account holder's name must be registered in Katakana letters. ※You cannot receive the Lump-sum Withdrawal Payment at the Japan Post Bank. 「銀行名」、「支店名」、「支店の所在地」、「口座番号」及び「請求者本人の口座名義」であることが確認できる書類を添付してください(銀行が発行した証明書等)。なお、日本国内の金融機関で受け取る場合は、口座名義がカタカナで登録されていることが必要です。 ※ゆうちょ銀行では脱退一時金を受け取ることができません。
Q: will a simple bank statement that includes the above be enough? Or have they asked for a specific form from the bank in the past?
④ Your National Pension Handbook and other documents verifying your Basic Pension Number 基礎年金番号通知書または年金手帳等の基礎年金番号を明らかにすることができる書類
Q: will a copy of the page that includes my pension number in the blue booklet be enough? Or do they expect me to send the booklet itself?
