Medical Insurance in Japan

I have medical insurance from my home country that I can use while living in Japan. To submit a claim, my insurance company requires an itemized billing statement from the doctor or hospital that includes:

• Provider’s full name and medical title
• Office address
• Billing address
• Phone number
• Dates of treatment
• The medical conditions or services I received care for

Has anyone gone through this process in Japan before? How would I go about requesting this from a Japanese clinic or hospital? Is there a specific document or Japanese term I should ask for?
