I'm insured via my company. Does that mean I should just submit my Insurance Card (a paper with the front and back scanned) plus pay slips showing that my salary has deductions for health insurance?
Normally I'd ask HR but earlier she wrote that she has sent all the documents the employer needs to send and was really condescending about it. Plus she has added a bunch of higher ups to the chat and I'd rather look like a fool on this subreddit than at work since this seems to be so obvious to everyone even though I can't for the life of me find the answer (except answers from AI which I don't want to rely on blindly.) (This would be the last document I need for my renewal, so I'm not going to hire a lawyer for help with this.)
