Lost Zairyu, instructions unclear.

Lost my residence card, I saw online that I have to go to a koban, report it missing, then go to a proper police station and get a report, then take that to immigration to get a new card printed (I'm in Tokyo).

I went to the Koban and they gave me a piece of paper that has a report number on it and told me to take it to immigration as proof of report.
Does this mean I don't have to go to the proper police station for this mysterious third step? Can I take the Koban form right to immigration or am I missing something?

Also, does it cost anything to get the new card issued?

Thank you!

  1. I don’t think you need to go to proper police station. I lost my zairyu card once and only koban slip needed. It was a long time age. So I don’t know rules have changed since.

  2. I lost my wallet and zairyu card a long time ago. I went to the police station and they gave a report as proof.
I don’t remember going to the koban. After then I just went straight to the Tachikawa immigration and got a new one (but had to wait for long hours). The sad (and funny) part is that they found my wallet and card a few weeks later and returned them to me after I had already gone through all the trouble of cancelling my credit cards and renewing everything.

  3. police station to report lost/stolen. Get their proof/cert. Get passport from home if you haven’t had time yet (though I assume police would want it for the report) as you will need to carry this + the thing from police until you get your new card. Go to immigration to get new card.

  4. When I lost my card I went to a koban and was instructed to go to the local police station.

    I don’t think there’s a cost, other than taking a photo.

  5. You dont have to go to the main police station as long as you reported the incident and the police gave you the 受理番号/acceptance number, thats all you need to fill up your 陳述書/statement.

    go here:

download the 陳述書/statement. Also, there is a checklist on what do prepare for application of reissuance of your residence card.
